Medical Supports

The Medical Supports component of the Family, Infant and Preschool Program is divided into three areas: Medical Home, Prenatal Supports, and Hospital-Based Services. The overall purpose of this program component is to provide resources and supports to health care providers to ensure that families and children have access to a wide variety of home-based and community resources.

 

Medical Home is an American Academy of Pediatrics initiative that FIPP is implementing with local pediatric practices to establish a partnership between physicians, families, and nonmedical professionals that promotes access to specialty care, educational services, therapy services, out-of-home care, family support, and other public and private community supports and resources.

 

FIPP participation in prenatal support activities involves collaborative relationships with obstetricians to provide support to women who are pregnant and whose unborn children are at-risk for poor developmental outcomes.

 

The hospital-based services offers FIPP as a resource to provide follow-up supports both in the hospital and upon discharge to families of children born at-risk for developmental delays or disabilities.
